Key Responsibilities:

• Provide comprehensive social work services to hospice patients and their families.
• Deliver care based on physician orders and individualized care plans.
• Work under the direction and supervision of the Clinical Manager.
• Support patients with emotional, social, and community resource needs.
• Collaborate with interdisciplinary team members to ensure high-quality end-of-life care.

Qualifications:

• Master’s degree in Social Work (MSW) from a CSWE-accredited program.
• Minimum of one year of experience in a healthcare setting.
• Current licensure in the State of Michigan.
• Ability to pass criminal background and MVR checks.
• Completed health statements confirming the ability to perform job duties.
• Valid driver’s license.
• Proof of current automobile insurance.
• Proof of a negative TB test.
