Job Description

A Path of Care Hospice in Norman, OK is seeking a PRN Medical Social Worker (MSW, LCSW) to support patients and families with compassionate, skilled social work services. What You'll Do as a Medical Social Worker

As a Medical Social Worker, you help patients and families navigate the emotional, social, and practical challenges that accompany serious illness. Your responsibilities include:

  • Assessing social and emotional factors affecting patient health
  • Participating in the development of individualized plans of care
  • Maintaining accurate clinical and progress notes
  • Identifying and coordinating community resources to support patients and families
  • Assisting with discharge planning
  • Attending in‑service trainings as needed
  • Consulting with interdisciplinary team members to ensure well‑rounded patient care
  • Practicing infection control and following universal precautions
  • Recognizing and reporting any signs of abuse in accordance with policy and law
  • Assisting with office tasks and new‑staff orientation when needed

Your work helps ensure patients receive holistic, compassionate care that supports both their medical and emotional needs.

Qualifications

  • MSW Certification
  • At least 1 year of social work experience in a healthcare setting
  • CPR certification or ability to obtain within 60 days of hire
  • Strong verbal and written communication skills
  • Excellent organization, time management, and problem‑solving abilities
  • Ability to remain calm and professional in stressful situations
  • Friendly, compassionate demeanor and ability to work well independently and as part of a team

Work Schedule

  • Full‑time
  • Monday-Friday, 8:00 AM - 5:00 PM
